Tax Planning

Below are descriptions of the IRS forms that we issue and links to answers to common questions about them. For more specific information on the forms or your obligations regarding them, please consult your tax advisor or the IRS.

1099INT – Interest earned on death claims, interest on proceeds, interest on dividends due to surrender, lapse or maturity, interest earned on dividend accumulations and premiums paid in advance. Review 1099-INT Tax Form questions.

1099R – Withdrawals, full surrenders, maturities, death benefits, taxable gains of surrendered and matured traditional policies, taxable gain on withdrawals from interest options, annuity income payments. Review 1099-R tax form questions.

5498 – Regular and rollover contributions on individual retirement arrangements, including Traditional IRA, Simplified Employee Pension (SEP), Savings Incentive Match Plan for Employees (SIMPLE). Review 5498 tax form questions.

W-9 Tax ID/Certification

Penn Mutual is required to report certain financial information regarding your business to the IRS. This form provides us with your correct Taxpayer Identification Number.
